Hands on Medical Stewardship
Ongoing, active care management for complex or evolving medical situations.
Includes everything in Foundation Care, plus:
Coordination across multiple specialists
Visit preparation and post-visit synthesis
Medication and treatment plan review
Family communication and decision support
Anticipatory planning and escalation guidance
Best suited for:
Cancer care, neurologic disease, multi-system conditions, or situations requiring frequent reassessment and coordination. -
Single Point of Clinical Accountability
High-touch care management for families requiring a trusted clinician to oversee all aspects of care and coordination.
Includes everything in Core Care, plus:
Proactive monitoring and escalation
Complex transitions of care
Coordination with family offices and advisors
Same-day response availability
Customized reporting as needed
Best suited for:
High-net-worth families, out-of-state decision-makers, and rapidly changing or high-acuity medical situations. -
Some services fall outside standard retainer scope and are discussed during intake.

Founder & Clinical Director
IntegrateHealth Care Associates is led by Margarita Sarabia, a board-certified nurse case manager with more than 20 years of experience across academic medicine, specialty care, and complex care coordination.
Margarita’s background includes UW Medicine, Swedish Medical Center, and other leading healthcare settings. She brings deep expertise in clinical care delivery, healthcare systems, and insurance navigation—allowing her to anticipate barriers, streamline coordination, and support informed decision-making.
Her work is most helpful when care involves multiple specialists, evolving diagnoses, or transitions that require trusted oversight and thoughtful coordination.
Collaboration & Contract Support
In addition to direct client work, IntegrateHealth Care Associates collaborates on a contract basis with healthcare organizations and specialty practices seeking experienced clinical leadership, care coordination, or advocacy support—when scope and alignment are clearly defined.
