FAQs

What is an independent nurse case manager?

1

Independent nurse case management provides clinical oversight, coordination, and guidance outside of medical visits.

As an independent nurse case manager, we help individuals and families understand complex medical situations, coordinate care across providers, and ensure care plans are carried forward thoughtfully between visits and across systems.

We work alongside physicians and care teams to support continuity and clarity — not to replace medical care.

How are services structured?

Do you offer hourly services?

Is this service covered by insurance or Medicare?

Physicians and care teams focus appropriately on diagnosis and treatment during clinical visits.

Independent nurse case management supports the areas between visits — where care often becomes fragmented — including follow-through, coordination, preparation, and family communication.

This additional layer of oversight helps ensure important details are not missed when care involves multiple providers, transitions, or complex decisions.

Who typically benefits from your services?

Clients who benefit most include:
  • Individuals with complex or chronic medical conditions
  • Families coordinating care for aging parents
  • Clients navigating cancer, neurologic, or multi-system disease
  • Families managing care across multiple health systems
  • Out-of-state family members seeking local clinical oversight
  • Many of our clients are balancing significant personal or professional responsibilities and value experienced clinical guidance.

How do we get started?

We collaborate respectfully with existing care teams and support physician-directed care.

We do not duplicate services or interfere with treatment decisions. Communication is professional, thoughtful, and aligned with the patient’s established providers.

The first step is to submit a consultation request.

We will review your inquiry and, if appropriate, schedule a brief conversation to determine fit, scope, and next steps.
Services are typically structured through a monthly retainer, reflecting the ongoing responsibility and clinical oversight involved.

The level of support varies based on complexity, scope, and desired involvement. All engagements begin with a comprehensive intake to ensure appropriate fit.

Limited hourly or short-term consultative services may be available in specific situations.

However, most clients benefit from an ongoing engagement, which allows for continuity, anticipation of needs, and proactive support.

Independent nurse case management services are not typically covered by insurance or Medicare.

Clients engage services privately, allowing for independence from payer restrictions and the flexibility to focus solely on the client’s needs.
